You have been called to a local daycare for a 4 year old child complaining of difficulty breathing and a severe sore throat . Upon arrival you find the student in a tripod position, drooling.
PRIMARY ASSESSMENT
LOC- A&Ox4, Irritable, restless
A- closing, stridor upon auscultation
B- SOB/ dyspnea/ suprasternal/ intercostal retractions
SPO2- 90%
C- tachycardia
CTC- hot, pink, diaphoretic
SECONDARY ASSESSMENT / HISTORY TAKING
O- sudden onset/ about an hour ago
P- tripod position

T- Getting progressively worse
S- muffled voice/ severe sore throat/ drooling
A- none
M- none
P- premature birth
L- oatmeal for breakfast
E- napping
VITAL SIGNS
BP- 102/66
HR- 128
RPM- 32/ labored
TEMP- 103.2 F
SUGAR- 138 mg/dL
INTERVENTIONS / REASSESSMENT
RX- blow by high-flow O2/ position of comfort/ KEEP CALM/ code 3 transport
REASSESSMENT- SpO2 improves to 95%
