It’s my very sincere and correct Head Canon that when Dean went to the empty to save Cas after his confession (which happened in the true and correct version of supernatural in all our minds) that when he pulled Cas out of the goo Cas was just looking at him; all shocked and mortified like
“Dean...what are you doing here? Why have you come for me?”
and then Dean just looks at him so softly and says
“What’s the matter Cas, you don’t think you deserve to be saved?”
