Headcanon A: what I think realistically
Elena stops taking her pain medication about as soon as they stop reminding her. By the time her birthday rolls around even the lingering headaches have largely gone, but her birthday may just be the most painful thing since those first few days after the accident when it sunk in that her parents weren’t ever coming back.
They try, her friends, they really do, and part of Elena is so so grateful for the way Caroline presents the cake and Bonnie whispers they even got booze but they’ll have to wait for a moment to sneak away while the (responsible) adults aren’t looking.
Matt stops by and it’s awkward, but he doesn’t push, even though he looks like he wants to. Elena doesn’t know how to tell him that “I need some time,” was already turning into “it’s over” the moment she said it. It wasn’t a total lie though. With all that’s happened, she can’t even imagine trying to deal with Matt and their relationship too.
Jeremy comes back in while her friends are already there, along with aunt Jenna and uncle John. His eyes are red, as if he’d been crying. He stands at the doorway for a long moment, staring at nothing in particular, or perhaps at the nothing where there would have been someone just a summer ago. Then, he turns abruptly and rushes up the stairs. They can hear the slam of his door even from the living room. There is an awkward moment where no one seems to know how to respond until Elena pastes a fake smile on her face and suggests they cut the cake.
Jeremy does not come downstairs again that night, not even for dinner, but when she goes to bed later she finds something on her pillow. It’s a pen, a fancy one, like the one her Mom had given her when she was eight to write stories with. It comes with a small card, which simply reads: Sorry. Happy birthday.
She fills nearly her entire journal with that pen before the summer is out.

I know the Vampire Diaries primary focus is the romantic relationships of the main characters. Its one of the things I love about the show. But watching the earlier seasons I can't help but wish that there had been a little more focus on Jeremy and Elena's relationship. I wish we'd seen more of protective-little-brother!Jeremy and big-sisterly-advice-giving!Elena. I wish we'd seen more of them.
