Last year, the Aesthetic Society released the first season of Beyond the Before & After. Episode 4 featured a patient of mine who lost over 100 pounds and dramatically changed his life. Season two is about to be released, so now is a great time to catch up on season one.

A study by the American Society of Plastic Surgeons (ASPS) concludes that routine measurement of hormone levels and an assessment by an endocrinologist isn’t needed for most adolescent boys with gynecomastia (male breast enlargement). The study was published in Plastic and Reconstructive Surgery®, one of the most respected, peer reviewed medical journals dedicated to the field of plastic surgery. So, what did they find?

A few weeks back, I announced the release of the Aesthetic Society’s Beyond the Before and After. This week, a reception was held at the annual Aesthetic Society meeting in San Diego. It was impressive seeing the larger than life sized pictures of my patient featured in the center of the San Diego Convention Center’s Exhibit Hall. Three of the four Plastic Surgeons participating in the Aesthetic Society’s Docu-Series, Beyond the Before and After were in attendance at the annual Aesthetic Meeting. This year the meeting was held in sunny San Diego, and for the first time in three years, I was able to attend in-person. Colby lost over 100 lbs before we met, but was still unsatisfied with the appearance of his body. Despite countless hours in the gym, he retained loose skin on his lower abdomen and had a saggy chest. He underwent a Mini Tummy Tuck with Liposuction and Male Breast Reduction with excision of the excess chest skin and relocation of his nipples. This is not the usual Gynecomastia Reduction. Not only was the residual breast tissue removed, but along with it all the loose overlying skin. In Colby’s case the loose skin included his nipples, which were saved and relocated as skin grafts back onto the chest. Last week's video shared the experience of my patient from before considering plastic surgery to the results. It has been well received, and people have asked to see more before and after pictures. So this week, I give the people what they want. This week we will examine the results via more Daddy Makeover Before and After Pictures.

The Daddy Makeover tightens both the chest and the abdomen and is tailored to each patient’s needs. In today's post, the chest reduction is more extreme, while the abdominoplasty is less extreme than average. Liposuction was added to maintain proportions.  Overall, the result is both an improvement in contour, and a new level of self-confidence for my patient.

Overdeveloped male breast tissue can be surprising and upsetting. The most common age for this occurrence is during puberty, when hormone levels change, but it can also occur in adulthood. Whether it’s on one side, like the man shown above, or on both sides, Gynecomastia is a source of frustration for men both in and out of clothing. Fortunately, treatment is available in the form of Gynecomastia Reduction Surgery.

Do you struggle with Gynecomastia? . This patient, in his early 20's came to see Dr. Romanelli for a gynecomastia reduction.  He presented with desire to have a more athletic appearance of the chest. Unhappy with "breasts" that were resistant to diet and exercise. Examination demonstrated that there was a great excess of breast tissue and we recommended that we perform both liposuction and a surgical excision of this excess tissue. Surgery was performed in our ambulatory surgical suite in approx. 2 hours(with a private nurse by his side).  Wore compression shirts to flatten the chest contour for 10 days. Photos below are two months after gynecomastia reduction surgery. His postoperative photographs show a great improvement. He is now comfortable taking his shirt off at the beach.
