It came in the JP version of the ToS Chronicles CE. They're apparently including the book in the NA version, too, which is up for pre-order at Amazon and GameStop as well as Namco-Bandai's store (but I don't trust them--the same dipshits that caused the Ni no Kuni nightmare are involved again for some reason).
