It's interesting to note that initially, the villain of Higurashi was supposed to be Irie. This explains why Onikakushi has a different vibe from the other chapters.

Like, in this arc, Keiichi is spied by the mountain dogs. They try to abduct him, like they abducted and killed Satoshi.

But by chapter 3, Ryukishi had changed his mind. Irie was a good guy and the final boss was Takano. The mountain dogs don't follow Keiichi after he flies from the clinic. (And when the extermination of Hinamizawa is started they don't go aften Keiichi.) And in Kai, it's revealed that Irie saved Satoshi's life (and that the mountain dogs weren't involved).

So yeah, the whole "I'm followed by people who want to kill me" thing was real in Onikakushi. It wasn't just Keiichi's paranoia. But this plotline vanished in the following chapters, and the plans for the disappearance of Satoshi changed.
