She wished to undergo reduction in the size of her lower jaw. Her parents brought her to our hospital for management. She underwent presurgical orthodontics to decompensate her arch. Bilateral bone cuts were made on the outer cortex and the inner cortex was fractured with an osteotome. Extreme care was taken throughout the surgery to ensure that there was no injury to the inferior alveolar nerve. The nerve was mobilized with the proximal segment. A slice of bone 6 mm wide was removed bilaterally. Occlusion was checked and the mandible was fixed with plates and screws.
