I’d say generally metaphor because they tend to be more interesting but depending on the subject matter straightforward can sometimes be more affective.

ooh, that must hurt. did you get all 4 removed? and did you get laughing gas?

Doesn't hurt at all actually, I am off of all drugs, no bruising whatsoever and I don't need ice any more. And it only happened yesterday. I was super lucky cause I had a surgeon who did a really good clean job. Ya i got all 4 removed. nope they just put me under.
