Why Can’t We Be (fb) Friends?
San Mateo County, California encompasses a large portion of the peninsula south of San Francisco, an area which includes the ultra wealthy right next to lower-income, working class folks. Many long time residents are being pushed out by the tech industry, who’ve capitalized on the space available outside of the city by created mega-campuses where their employees spendeverywakingminute work. While tech workers complain about health issues, they also complain about seeing homeless people on street (SF Gate, 2016). This county has gone through a huge transformation in recent years, but not without health consequences and continued health disparities.
A brief analysis of San Mateo Health System (SMHS) (yes it’s called system and not department), showed that despite having a presumably ample influx of funding for the department, and despite being located in the tech center of the universe, SMHS’ facebook page could use some serious improvements. SMHS scored a 51 on likealyzer.com (#NotASponsoredPost), lower than the average Government Organization score.
The Good
- Their posts are well timed for their audience, btw 8 AM and 11 AM -- (I thought techies didn’t get to the office until 11 AM)
- They have 15,118 likes -- SO POPULAR!
- #HashtagsOnPoint #TheyGotThis #Winning
- Asking fans questions -- showing they really care about your thoughts and opinions
- Allowing everyone to post on their timeline -- open borders just like Hill
Real Talk
- Growth -- SMHS could do a lot to help bring new followers/likers. While they have a large following for a Heath Department, it represents only a small portion of the population. Perhaps providing more visibility on their shiny webpage, or including a facebook link on materials used in the community. Also, paying for sponsored posts. The more visibility the facebook page has, the greater chance SMC’s residents will get access to useful health information.
- Milestones -- not everyone likes to celebrate their birthday (only me?), but having milestones on facebook could help user engagement, especially when it comes to a milestone such as the number of flu vaccinations received or success of a teen pregnancy campaign. We often focus on the bad stuff in public health, but celebrating successes (be they small), can create a more open and inviting page to browse.
- Engagement Rate -- Similar to growth, creating engaging content and finding ways to actively engage users through facebook could seriously help residents. During times of crisis, maybe an earthquake or concerns over Zika, most folks turn to the internet to find info. The better engaged SMHS can become with their population, the more likely folks will get accurate information. Maybe stream a facebook live session where residents can tune in and ask questions to health workers in real time.
- Posts per Type -- this feeds into engagement, in some respects. The more variety you include in your facebook posts, the more interested and engaged folks will be. Adding videos, especially in the style NowThis News has done, are an effective way of getting important, easily digestible information out to a wide audience. These posts also encourage more engagement, which leads into the last point.
- Response Rate -- SMHS was rated “catastrophic” for their response rate. Ouch. It’s wonderful to ask questions and post fun infographics and posters on facebook, but if you’re not responding to residents, especially those in search of health information, you’re pretty useless. This is why municipal offices get a bad rap. Reply to people, help them, it’s your job.

Detroit, Michigan has also undergone enormous transformation in recent years, but in the opposite direction of the Bay area, with the collapse of the auto industry. Detroit was also recently implicated in the Flint, Michigan lead crisis when the city of Flint switched their water supply from Detroit’s Water and Sewage Supply, to their local Flint River. Detroit was spared much of the issues with lead, but it exposed a break down in communication with local health departments and larger governmental entities (ie. EPA) and lack of attention to standard safety precautions (npr.org, 2016). Despite mostly avoiding the issue of lead, Detroit remains underserved as the city’s population has dwindled, incomes have decreased and basic infrastructure and municipal services have broken down (New York Times, 2012). This sort of downturn has huge implications for health and only exacerbates health disparities for residents.
A brief analysis of the Detroit Health Department (DHD) showed that despite report of failing municipal services, the DHD’s facebook page performed better than San Mateo! DHD’s facebook scored a 70 on likealyzer.com, still not an A but higher than the average Government Organization score. You go DHD!

Real Talk
- Timing -- DHD could do a better job of timing their posts. Most are posted between 2 PM-5 PM (EDT) when they should be earlier 5 AM-8AM (EDT). Detroit folks are early risers!
- Milestones -- similar to SHMS, milestones can be really important in health. Boosting user engagement, by celebrating successes. People don’t want to read depressing news, but they do love a good dopamine hit (read: kitten videos) with positive, feel-good stories.
- Likes -- Detroit is a city in need of better communication, especially considering the dwindling population and state of municipal services. Seeking a larger audience, more of Detroit’s population, could help visibility of DHD’s services and help address some health disparities. Facebook offers sponsored posts and other ways to boost audience, do this.
- Posts by fans -- Though DHD may be well-liked, there are few posts by fans. DHD could get more posts by branding their facebook page on DHD materials, encouraging facebook likes/engagement at DHD events, and making DHD a Q&A center for Detroit residents.
- Curiosity -- Similarly, DHD could engage fans more by asking them questions. Poll about health issues important to them. Ask residents who’s received their flu shot. Providing a platform for users to respond, opens up communication between residents and DHD, and can actually inform the sorts of initiatives DHD would like to start for residents.

For San Diego
The top post for San Diego, according to Likealyzer are mostly posts that give people things to do, which is similar to that for San Francisco. People like when they hear about events they can attend in their area that they otherwise might not have known about. The interesting thing about the San Diego page is that people also seem to be interested in their job postings. This is an interesting thing for the San Diego page to know and could help them decide what to post about.
This page seems to be more active in general. While having fewer followers, the posts are more frequent and have a higher amount of engagement with the audience.
5 things they are doing well
1. posting frequently
2. posting images
3. posting about timely, important matters when they happen
4. posting a series, which can help build an audience
5. Replying to people who comment on their posts
5 area to improve
1. when they post about an important event, they should cancel (or reschedule) their scheduled post. Looking at 10/24/16 there are multiple times during the day where two posts when up almost simultaneously. The time sensitive posts should stay, but they should be aware of what else is there.
2. Not posting the whole series when the do post a series (if there are 10 videos – “Earn the perfect 10 of Preparedness”) I think for 10 days those should be the only posts (1 a day) unless an emergency also comes up
3. Linking to other groups pages to build a network
4. Post information from other sources that are not just countynewscenter.com
5. Figure out when most people engage with the content and time the posts for the most views – this would most likely be later in the day than when they currently post
For San Francisco
The top posts according to Likealyzer were almost all “on tap this weekend.” Looking at what they do post, this makes sense. Most of their posts are informational, but the ones about “on tap this weekend,” while still providing information, also give people ideas for fun things to do. Otherwise, most of the posts are about things to keep you safe or be more practical. While those facts are things people should know, Facebook probably isn’t the venue for that information – it would be better placed on their own website.
They also do not post frequently or regularly, making the times the do post even more important. If they are posting infrequently, I would suggest that what they post be vitally important information. Right now, it seems like they try to post about once a week because they know they have to post (although in the month of October the posts have gotten more frequent).
