511 Kinderheim was run by fear avatars, and the children who lived there were raised to serve the Fears
It is very unclear which entity Johan serves. Some think he’s an avatar of the Slaughter because of the incident at 511 Kinderheim. Some think he’s an avatar of the Spiral because of the psychological torment he instils in his victims. Even more believe he serves the Stranger, the Web, the Eye, even the End. Nobody has ever been truly sure.
Many fear avatars (like Roberto, who is an avatar of the Hunt) try to use Johan to aid in their rituals to bring about their own apocalypse. Johan sabotages them every time, because he is the only one who should be allowed to torment the world.
The truth is that Johan is not an avatar at all. He does not need an entity’s powers to instil true fear in someone. In a way, he is his own entity.
The main story of this AU is structurally similar to Monster. Tenma is tracking down Johan to fix his mistake and prove his innocence. As he goes on though, he slowly becomes an avatar of the Hunt. He keeps failing to kill Johan, at first out of fear of losing himself, but soon he realises that he does not want the hunt to end. Some sick part of him enjoys tracking him to the ends of the earth with the intent to kill him, and that part of him may take over completely if he lets it. Dieter does a good job at keeping him away from the edge, but who knows how long that will last.
Other avatars soon realise that Johan is a detriment to their plans and rituals, and he needs to be taken down. As Tenma tries to kill him, the other avatars work together on a new ritual. They mark Nina with all the fears, like Jonathan Sims was in TMA, in order to use her as a conduit to bring about the apocalypse.
Grimmer is an avatar of the Stranger.
Lunge is an avatar of the Eye, but he is unaware of this fact until it is much too late.
