so what would the gender ratio of itheia's children be? and how would they be treated by the maidens? also, are the maidens allowed to get married to each other/outsiders?
I honestly don't think the gender ratio would be affected much. The Maidens would assist to some degree in her child's care while they grow up and remain at the temple. It'd be added to their list of duties and considered as honoured a task as seeing to Itheia's daily needs directly.
Maidens are absolutely allowed to marry one another or anyone else they please. Their partners live with them in their quarters for as long as they remain Maidens. Typically, people who aren't women are not permitted to live in the temple (they can visit and offer worship/gifts, but can't reside there), but Itheia makes an exception for the partners (and children) of her Maidens; she highly values the happiness and freedom of her devotees. So, even if they don't marry women, they can still be with their families while keeping their Maidenship.
However, a rule does remain that people who aren't women or girls aren't allowed in the temple's inner sanctum.

Certainly! As stated in her story, Itheia is a divine protector of mortal women who resides in a temple in the human realm and is attended to by her Maidens. Maidenship is a voluntary path taken by women and can be a lifelong position if one chooses. The temple is equipped to house up to thirty Maidens at a time.
Itheia's temple is a sort of spiritual extension of her, so acts of service to it are how the Maidens go about their worship; keeping it clean, tending the gardens, creating art to be displayed there, performing songs and ritual dances within its walls, etc.
Women and girls who aren't Maidens are of course permitted to visit the temple and offer prayer, gifts, or take part in the song and dance. The gifts range widely as Itheia accepts most tribute. They mostly come in the form of food, jewellery, and clothing but also (and these are typically her favourite) items that hold a more personal rather than monetary value to those who are gifting them. She has a collection of dolls and other toys from girls who treasured but outgrew them; they radiate the love given to them over many years that she just adores.
As for the second part of your question; yes, it is possible but not likely in that particular story. The gods I write about who have uteri can actually choose if they get pregnant or not from unprotected sex. They have conscious control of their fertility; from entirely nonexistent to high. Just another perk of godhood which helps given their penchant for excessive physical indulgence.
I do like to think that Itheia has a handful of demigod children, but they would be rare seeing as she's a lesbian and, statistically, trans Maidens are few and far between.
