So the resident who kept pimping me and making me feel like a dumbass gave me feedback today which basically said "I rode you hard because I knew you were really smart and you did a good job." Which was like, so much nicer than I expected? Good surprise. I also got nice written feedback from 2 of my attendings so that was good :) Bad surprise: discovered that a bunch of my colleagues in the ICU are republicans 😒 Yesterday was such a rollercoaster. On the one hand, my patient who I've been looking after for 11 days ever since he went into a coma (severe hepatic encephalopathy) is finally awake, extubated, and talking. He's totally confabulating though, and it's legit hilarious. In the middle of my exam, he looks me dead in the eyes and says "You're a hot mess." I literally looked at him and said "Dude, I'm the hot mess? *I'm* the hot mess? Ok. Ok." 🙄🤣 he may have actually had a point with that one... On the other hand, my first legit "ICU" patient (aka, not a holdover I saw on medicine like Mr. Hot Mess) passed away overnight. It was only his second day in the ICU, but we did a mini M&M and my team thinks it was septic shock leading to type 2 MI with subsequent cardiogenic shock. We gave him tons of pressors but his pressures still tanked. We should have tried dobutamine, it couldn't have hurt... I'm enjoying ICU so far but I don't think I could do this. It's too heartbreaking. And I hate not knowing my patients. More and more I'm getting the idea that I'm not an inpatient physician at heart. I love seeing my patients every day and being able to track their progress, but it's hard for me to let go sometimes when I transfer service or they get discharged. I want to know the end of the story, or at least, what happens next. Maybe I can be one of those old school docs who comes to the hospital to admit the patients they see outpatient...
