Equine Ulcers - Aloe Vera vs Omeprazole — Head-to-Head!

Background: One of the most common recurring issues faced by horse owners and trainers today is gastric ulcers, otherwise known as “equine gastric ulcer syndrome” (EGUS). Gastric ulcers in equines can result in a variety of negative outcomes including behavioural issues, discomfort, feed intake concerns, and overall inability of the equine to thrive.

Omeprazole is a commonly prescribed medication in the treatment of ESGD in horses and it is the current “drug of choice” for this condition. Omeprazole is a proton-pump inhibitor that is absorbed into the blood stream and affects the acid-producing cells of the stomach to reduce the protons being pumped into the stomach, thereby increasing the overall pH of the gastric environment. However, ulcer redevelopment is common in horses following the end of treatment with omeprazole, even with changes to equine management.

The association between aloe vera and gastric ulcer prevention and even treatment in horses has been known colloquially and anecdotally for many years. Oftentimes equine professionals will recommend adding aloe vera to a horse’s ration when the animal is beginning a long-term course of NSAID treatment, or during other stressful situations such as travel, showing, etc. While the positive effects of aloe vera on ulcerative conditions have been well-documented in human nutrition (Palmieri et al. 2019; Picardo et al. 2020), the association between aloe vera and ulcer prevention in horses has never been directly tested in a controlled study before now.

Study Design: A 2018 study in the Equine Veterianry Journal directly compared the efficacy of aloe vera (inner leaf gel powder reconstituted to a paste) and pharmaceutical grade omeprazole. Horses were confirmed to have EGUS and then randomly assigned to a treatment group. There was no negative control as the researchers determined that would be inhumane. Horses received twice daily (aloe vera) or once daily (Omeprazole) treatment in accordance with a manufacturer instructions, for 28 days and then ulcer prevalence and severity were reassed.

Results: Overall, results showed that aloe vera was “not noninferior to omeprazole”, meaning the pharmaceutical still showed greater ability to improve and heal EGUS than the aloe vera intervention. However, 56% of horses saw some degree of improvement on the aloe vera treatment (compared to 85% in the omeprazole treatment group). The results also indicated that for complete resolution and healing of gastric ulcers, omeprazole was far superior, with 75% of horses in the omeprazole group showing healing, compared to only 17% in the aloe vera group.

Because there is no chance an owner would volunteer their horse for intentional ulcer formation, there is very low likelihood of a trial for testing aloe vera as a preventative measure. However prior research in rats (Bora et al. 2011) did show a decrease in gastric damage with aloe vera pretreatment (though not as much decrease as rats pretreated with omeprazole).

Conclusions: In conclusion, while aloe vera is not the best treatment option for certain types of equine gastric ulcers, there is now some proof to back up the anecdotal claims that it does have at least a minor measurable effect when it comes to improving the severity of gastric ulcer lesions.
