40 S Halsted St: A large sunken area of pavement in front of the Mariano's and Stan's Donuts on Halsted. The sinkage is severe enough to catch large amounts of water. The significant amount of pedestrian traffic in this area causes even more concern for safety, especially its location in front of a busy grocery store.

If you come upon any sidewalks with raised pavement, chunks missing, or any other forms of damage, call 311 or visit 311.chicago.gov to report them.

200 S Halsted St: Significant cracks and missing chunks of concrete are present around a fire hydrant near a crosswalk. This crosswalk bridges between the restaurant Greek Islands to the Halsted Walgreens. Walking near the curb and crosswalk can pose a significant of tripping or falling. The damage being near the road and the presence of a solid metal fire hydrant causes significantly more concern for injury.

3316 S. Bell Ave.: This section of sidewalk is located on the west side of Bell Avenue, next to a residence, and the surface is uneven and raised. A concrete slab has been pushed upward, possibly due to nearby tree roots—creating a noticeable height difference between the sidewalk slabs. This uneven surface poses a significant tripping hazard to pedestrians. 917 S. Racine Ave.: This section of sidewalk is located on the west side of Racine Avenue, near the intersection with Taylor Street; it has severe cracking and is uneven. Damaged concrete and exposed asphalt contribute to the unevenness of the surface, creating a potential tripping hazard for pedestrians if they are not careful. 1233-1299 W. Cabrini St.: This sidewalks situation is less of a crack and more of an entire plate shift. Caused by the nearby growing trees root system, this sidewalks right side has completely elevated and is sitting at an angle causing those crossing to physically step or rise over the edge. A hazard to those on foot and wheels travel here is difficult for all due to the angle the concrete is sitting at.

813-999 S. Ada St.: On the right side of the right side of the sidewalk there is a notch. While this notch is less of a stroller and wheelchair issue it is the perfect angle and level for someone’s foot to catch mid step. Packed with dirt and debris when cleared lies a 2-3 inch notch guaranteed to have anyone stumbling.

115 S. Sangamon St.: This raised sidewalk is a dangerous hazard located along the west side of the park. Pedestrians with strollers should be especially careful to not get caught by the raised portion closer to the tree.
