Karlach why you gotta be that way?!

I've been romancing Astarion since the beginning. But I must have led Karlach on at some point, because that girl had been trying to get in my pants forever. Now we're at a point where she's all "fixed" and wants to bone. But I'm with Astarion. But, I can't be like bro I'm with him.

And if I sleep with her and talk to Astarion he's like you wanna be with me?! Well, Karlach doesn't share so you have to dump her. But she's so mad I didn't tell her. Ugh, maybe I'll just ignore her.

Does approval go up with Astarion if I dump Karlach? Will she hate me forever?

Update: So I had saved right before I spoke with Karlach so I went back and just turned her down gently. I did check to see if Astarions approval goes up if you choose him over Karlach and it doesn't. However I do love the cutscene. And you do make things "official" with Astarion if you choose him over her.
