Dean is not scared by many things at this point, he’s been a hunter for a long time now, and he’s pretty much seen everything by now. He’s no longer really afraid of the monsters he takes out; because, as a hunter, he does it on a day-to-day basis. He’s also not really afraid of getting hurt or killed, he’ll avoid it, obviously he doesn’t want to get hurt or die, and he definitely doesn’t want Sam or anyone else to do something stupid do help him but he’s not actively afraid of it.

The only real thing concerning his own well-being/state of being is becoming a monster; way back in season 3 we see that in the form of the first clip in this video, “you’re gonna die, Dean, and this; this is what you’re gonna become.”, this is dean’s worst nightmare it’s portrayed in this scene as such. That's why this season is gonna hurt me so bad, Dean has literally fulfilled his worst nightmare.
