Maybe Sam thought Dean was lost while he was with John? That’s what I thought they meant.

But John left Dean to take care of Sam when he went on the hunt. Sam even said that John got back and Dean had disappeared. I don't know. Maybe I'm reading too much into it, you guys. It may also help if I read John Winchester's journal, but I don't know it felt off to me. Not a huge deal though. :p
