Sam dies and goes to heaven.
He's ok with it, and he just hopes that Dean will make the best of it and have a good life.

Sam is talking to Bobby, he's so glad to see him again.
Boom bang poof
A massive earthquake and sounds of destruction.
"what the- what's going on?"
"don't know but it's gotta be one big problem if it's causing this in literal heaven!"

Big dark power demonic hands come out of the ground.
Sam and Bobby are running away, as well as everyone.
The angels have come to help but they're failing and being grabbed by those hands and almost crushed.
Then a hand come out of the ground right where Sam was running to, he didn't have enough time and he was grabbed.
That's when everything stops, the earthquake is slowly getting better and stopping, the hands completely froze in place.
Then the hand holding Sam starts moving and he's pushed through the ground and out of heaven.
He's worried and kinda scared when he arrives at a bloody forest, literally bloody.
"Sam! It worked!! Are you ok??? Are you hurt in any way??? Do you need something??? Don't you ever die on me like that ever again!!!!"
Dean...
"h-how..."
Apparently, Dean had provoked a certain celestial being until he was thrown into a different universe, one where he's a literal prince of hell, the son of Lucifer, created by some cultists.
With that power he brought heaven to its knees and took what he wanted, who he wanted, his Sammy.
Yes, this is a reference/crossover with devour I just love Jake.
And you can't tell me the concept of Dean being the true ruler of hell and not little 'boy king' Sam isn't interesting, I might even write some more about it!
