Was talking about this in the All-Zhan server but consider:

An au where Madam Lan escapes while pregnant with baby! LWJ and ends up leaving him at the doorstep of a brothel in Yunping and disappears into the ether. Anyways, great news for LWJ (he should be called something else in this verse but haven't decided what) since coincidentally another child had been born recently so he is put with the courtesan who had the child. Meng Yao and Lan Wangji grow up as milk siblings and fiercely protective of each other in the brothel, but LWJ had the misfortune of being born with a pretty face and doesn't really have a mother to vouch for him in the same way, so he is pulled into the business when very young, forced to please clients to earn money. This LWJ I imagine is a lot more jaded than canon LWJ, and looks extremely dead on the inside the whole time, only brightening up when he sees Meng Shi or Meng Yao.
