(Also inspired by winchester-kelly's fluffy ending. Though it made me smile, I still wanted to write this one... a bit more angsty. *shrug*)
The water was freezing. Dean thought it would make him numb, like when you cut off the circulation in your limbs, before letting up the pressure. Instead, it was like the aftermath, pins and needles with an overwhelming cold burn that just engulfs everything. His chest felt tight, like he couldnt take in a deep breath, but still he swam with one arm, holding onto the back of Cas's life jacket to guide him through the chaos of other survivors splashing about to get to the floating piece of wood he was determined to get Cas on and out of the water.
People screamed and cried around them, some hurt, others confused and in shock, all of them scared out of their minds. If he had any hope that kept him thinking clearly, positively, it was that he managed to get Sam to stay on the lifeboat. He was safe and now all he had to do was get Cas to be safe, keep him alive. "Almost there, Cas," he said gruffly, spitting out salt water when it lapped into his open mouth, "just keep swimming. Keep goin'."
They got to the broken, decorative wall that shifted over the agitated waves of the freezing ocean and he helped Cas climb on top. Cas instinctively moved over to let Dean on but Dean tried once and saw how the wood sunk too low and he stopped. He was too heavy. So, Dean moved around the edge of the makeshift life raft, still in the water. so that he was closest Cas's head and he offered a wavering smile that made his chin quiver.
"Wha- Dean, what are you doing? Get on..." Cas said through chattering teeth.
Everyone was still screaming, churning the water more, while one of the ship mates blew on his whistle to signal for help. Dean looked over to look at the older man as he whistled and tried to shout over everyone else but Cas grabbed the front of Dean's thin shirt in one hand while the other shakily cupped his cheek, trying to get his attention. "Cas, there's no room..."
"Bullshit, Dean, get on," Cas snapped back, his blue eyes almost black with how big his pupils were, a stark contrast to how pale Cas had become from the cold and shock.
"I'll just sink i--"
"Dean!" Cas sobbed, his voice cracking.
Despite the outburst, Dean remained stubborn and smiled again, his lips twitching as he blinked rapidly at Cas. "It's okay, Cas... 'sides, the boats will be here any minute now. Ya hear that whistle? It's gonna bring 'em over..." he assured and released the edge of the door with one hand to bring it to cradle the back of Cas's neck as he brought their foreheads together. "We're gon' be okay."
Except it wasn't just a few minutes. The whistling had stopped some time ago and the terrified screams from all those in the water had turned to quiet, despairing sobs. The water was calmer now and lapped lazily under the wood as they bobbed in the waves. Dean could hardly breathe anymore and both of them had hair that was more ice and snow than hair. Cas was shaking violently, still lying on his belly while they held hands and looked around their surroundings with dark circles under their eyes. To try and make himself better, Dean thought of sharing a warm bed with Cas, his dark brown hair tucked under his chin and tickling his nose while Cas told some profound story, his voice reverberated through Dean's chest, almost in harmony with his heart beat. Between the fond hope for the future and the peaceful lull of the ocean, Dean's eyes had started to flutter closed until Cas shook his arm violently and shouted his name.
"The boats 'ere?" Dean asked and lifted his head from resting on the very edge of the wall.
"N-no," Cas hiccuped and started to tug more on Dean's arm again, as if from his vantage point he could actually manhandle Dean into a new position. "N-no... b-but you're- you're f-f-freezing! Get on the-- on the damn ra-raft!"
"Too h-h-heav--y."
"Y-you s-s-said a f-f-few m-minutes. Times u-up. G-get on!" Cas tugged on him again and started to scoot over. When Dean didnt move and instead closed his eyes again, Cas frowned and slapped his cheek a few times. "I-if y-you don't..." he swallowed thickly and blinked harshly "get on, I-I'll get off," he threatened, which got Dean moving very quickly.
"F-fine! B-but if it s-s-sinks 'gain, I stay in the wa-water," Dean replied and edged along the board to get access to the side again.
When it almost capsized again, Dean had tried to give up but Cas yelled at him to try again until they could get the balance right. It wasn't about how much room was on the wall, it was about weight distribution. Eventually, they got it right, just as most people around them had quieted to only soft murmurs and sniffles. Dean had stopped shaking, his body was in such an advanced state of cold and shock, while Cas continued to shiver and shake.
With both of them on their sides, Cas pressed one of his hands flush against Dean's chest so he could feel Dean's heartbeat, it was slow but it was there. Dean pushed one of his legs between Cas's, his wet slacks creating friction between Cas's already frozen ones, and tucked his arms between their bodies. "Would i-it be ta-taboo o-of m-m-me ta complain 'bout n-not bein' a-able t-ta feel my t-toes?" The corners of his lips lifted a smidge but his joke had the opposite affect on Cas, who frowned deeply.
He looked around but there was still no sign of help coming. Cas was about to comment when he felt Dean's frozen hand clamp around his wrist and he turned to look at Dean, meeting his gaze, no matter how far away Dean seemed with that vacant look. "Wi-winning th-that ticket is the best th-thing that ever ha-happened ta me C-Cas," he said softly, blinking a couple times as tears tried to form but couldn't. "It brought me to you." His voice was stronger then, filled with conviction and promise.
Cas sniffled and bit his lower lip in an attempt to stop chattering his teeth. "I love you, Dean..." he whispered, a truth. A goodbye. No one was coming for them and he had let Dean stay in the water too long, it wasn't fair but this seemed very true to the end.
Dean seemed to accept this and smiled as best he could, slid his hand down from gripping Cas's wrist to curl his fingers around his palm to pull away from his chest and kiss his fingers. His eyes slid closed as he did so but he never let go, grasping Cas's hand even as he started to slip into sleep.
More time went by and now the stars were out but neither shifted their positions on the precarious piece of wood. The others no longer made any sound, nothing could be heard over the waves or Cas's gentle humming of a slow, soft song. He had yet to fall asleep so he had been periodically checking on Dean, getting him to open his eyes just to make sure he was still alive. Cas was about to do another check, shake his shoulder and call his name until Dean would open his eyes, when he heard distant yelling and saw a beam of light flash across the black, churning water.
He lifted his head to look past Dean's shoulder and gasped at the sight of a single life boat rowing by. "Dean," Cas whispered hoarsely and shifted on their 'raft' so that it bounced in the water. He shook his shoulder and called his name again but Dean stayed asleep, his eyes shut tight and pale lips parted. "Dean, there's a boat..." he whispered again, his voice lacking any true strength even though he had stopped shivering some time ago. "Dean, we're saved, wake up."
Still no response from Dean and the boat was going in the wrong way.
"Dean!" He shook him even harder, his voice getting higher in pitch as he began to panic. "Wake up!" Desperate to see Dean open his eyes again, Cas moved his hands from Dean's shoulder and chest to cupping both of his cheeks and shaking. Even with his numb fingertips and palms, he could tell that Dean was far too cold, possibly colder than he was. "Dean!"
His final shout for Dean to wake up did nothing to get green eyes to look back at him but the light from the boat swept over their way finally.
A deep voice called from the boat, "hello?!"
"Here... help..." Cas rasped and lifted one arm to signal where he was. "Help..." He waved but didn't lift his head to look at the boat, his gaze fixated on the still face of his love in front of him.
"We see you, hold on," the voice shouted.
He went back to shaking Dean frantically. "Dean, no... please..." Cas closed his eyes and pressed their foreheads together, their noses brushing against one another's in feather light touches. "Please, Dean, I need you... come back. I love you." The boat was closing in, Cas could hear the paddles in the water as the voice from earlier tried to get Cas to sit up or make himself more visible, but Cas couldn't let go, couldn't pull himself away from Dean.
"C'sss..." Dean greeted through barely parted lips and Cas pulled back, his eye flying open to stare at Dean in disbelief. Looking back at him were two green eyes. "Wha-?"
Just then, the boat made it up to them and men from inside tugged their raft closer and tried to get their hands on both Dean snd Cas to pull them inside. Before either could do or say much else, they were both sat on a bench in the center of the row boat with a single blanket wrapped over their shoulders so they could share both the warmth of a blanket and body heat. They shared a glance and then looked to their rescuers and nodded their thanks as they both simultaneously resumed shivering. They had been saved... together.
Later, they would be reunited with Sam, they would take the necklace and run away together, the three of them, running away from Cas's oppressive family and chosen destiny in favor of practicing free will and choosing their own future. A family of their own making would grow from three to over a dozen; a grumpy old man with a junk yard, a woman and her daughter that ran a bar together with an alcoholic outcast (the woman and grumpy old man would one day marry), two female cops with two adopted daughters of their own, a fiery, red headed lesbian that worked in burlesque, and a con man with his fabulous mother. When Dean and Cas do die, it's after a full life, safe and warm in their bed, survived by the children of the family they helped form, and with decades of happy memories.

So I just realized that titanic jack and rose are so alike to natsu and lucy. Granted the ending is sad but who doesn't love angst
