Anterior Calvarial Reconstruction for Trigonocephaly This little boy was born with a triangular shaped deformity of the head when viewed from above. He also had a prominent bony ridge on his forehead. His parents were very troubled by this and also by the fact that he was very irritable and defiant, even as an infant. A local hospital had referred them to us for management. Extensive examination and imaging studies had been performed at our hospital with the diagnosis of metopic stenosis with trigonocephaly. Subsequently, he underwent corrective surgery at our hospital. A bicoronal flap was raised followed by frontal bone osteotomy and lifting out of the frontal bone. Bone cuts were then made that allowed for complete separation of the supraorbital bar from the skull. Metopic ridge was trimmed and anteriorly advanced and fixed with sonic welding. This was followed by barrel stave cuts on the frontal bone to create space for brain growth. Frontal bone was then placed back into position and the bicoronal flap was closed with staples. The improvement in the little boy was dramatic by the 4th postoperative day. He became a very playful little boy and was constantly smiling at everyone.
