Um…so that episode brought up a terrifying prospect.
The citizens of Westview are very aware of what’s going on and they are not under any kind of spell or illusion. They’re just playing along with someone else’s sick version of house.
The first two episodes made it seem as though the citizens truly believed that this was their life, but with moments of them waking up or trying to.
But it seemed pretty clear that Herb (I don’t trust Agnes) is aware that they are all trapped and Wanda is the cause and the only reason the town isn’t surrounding the Vision residence with pitchforks is because their scared of Wanda (which yeah, totally understandable).
Though, how cognizant of the situation they actually are is still up for debate.
Even though the Doctor made the comment of not being able to leave a small town; he and his wife did seem to believe that they would be able to leave for their trip.
We even saw the homelife of Norm and Dottie, where they acted like a stereotypical sitcom couple when it was just the two of them (Norm has the best one-liners).
So, there are two options –
(A) The Westview Citizens always have to be or at least they believe they always have to be on.
(B) It’s a mix of them being aware and being under some ‘spell.’
In regards to Option A – Maybe the Doctor and his wife thought if they just pretended they were leaving for the Bahamas for a fun vacation, like they would in real life (sitcom shenanigans and all), then maybe they could escape.
Though I’m inclined to believe that overall Option B is the reality of the situation.
Considering how that’s the case for Vision – he’s aware of the ‘strange’ occurrences (to put it mildly), but yet he can be made to quickly forget. Even the situation of Wanda getting pregnant and giving birth in one day, which he knows is “odd,” he still just accepts it as reality.
Kind of like a dream. You don’t question the logic of your dream world, because you aren’t aware you’re dreaming.
Though as I finish writing that, I am realizing that just because that’s the case for Vision doesn’t mean it’s the case for the citizens of Westview, and Option A may still be veery much the case for them.
They’re real people.
While Vision is a construct of some kind (seeing how he died), so maybe it’s much easier to bend Vision to the will of this reality. But for the actual people – you can only bend them so much, before they break.
Though if you were fully aware of what was happening (being a prisoner of someone playing house) –
Would you actually confront the person you believed to be responsible as Dottie did or in the way Dottie did? Or continue cutting into a wall once you’ve been made aware like Herb?
So, maybe there’s an Option C
Someone is controlling the citizens of Westview and so while they are aware and can kind of break free, they are helpless. Whoever is controlling them isn’t able to control them all the time nor truly have 100% control.
I don’t think that person is Wanda though. While she’s looking less and less innocent (we’ll see how culpable she actually is by the end), I don’t think she is the one controlling the citizens. She can affect and manipulate this so-called reality, but it doesn’t seem like she’s controlling the people like a puppeteer.
And remember, she didn’t know her and Vision’s story when Mrs. Heart asked them. Just like Vision.
Clearly if she was fully in control, she would’ve just lied and have Vision believe the lie himself. Or something like that.
And furthermore, wouldn’t she have also brought back her brother if she was truly in full control?

So yeah. In general, this episode was the most horrifying to me. Just the mere idea of the citizens being that aware of what’s going on is so much more terrifying (to me), than them being under a spell with moments of cognization.

Also, I think Agnes is definitely part of whatever is going on.
My current theory – is that maybe she’s letting Vision be aware of what’s happening (bit by bit), to cause a confrontation between him and Wanda.
Therefore, she’ll have an opportunity to steal the twins and Wanda will take all the blame.
