Do you think Sam and Dean ever got caught by a teacher?
No, simply because of the logistics. My brother and I have the same age gap as Sam and Dean. We were in the same school once: when I was in 4th grade and he was in kindergarten. Sam and Dean presumably also had 5th grade and first grade, and maybe even 6th grade and second grade, since middle school starting in 5th grade very much wasn't the thing back then, but that's still much too young to be really caught doing anything. I don't think they were being all squishy and romantic as kids. They weren't normal about each other, not even close, but not overtly romantic. I don't think you can really give siblings shit for kissing as a teacher at that age, you know? If their family is like that, you can't interfere.
So by the time they're old enough to do anything that would get them in trouble, they're in different schools. Sam cared way too much about school to let Dean sneak in to fuck him, and he wouldn't cut class to do it anyway. As hot as that would be and as much as I'd love to read that porn, it just isn't plausible. Is Sam extremely horny for his brother? Yes. But we know for a fact that the one thing more important to him than fucking Dean was getting the hell out of the life. So, unfortunately, no in school sex for them to be caught doing.
Now, do I think things happened in the parking lot when Dean picked Sam up? Yeah, obviously. But I don't think teachers really gave a shit about what was happening in parked cars. None of my teachers ever watched the parking lot, anyway.
That said, I am a fan of the classic premise where a teacher pulls middle school Sam aside, having seen that he's covered in bruises, clings very close to his brother, and is generally weird about him, and asks if he's being molested. That I 100% believe. But that's not actually getting caught in the act, you know? And besides, they're wrong. The bruises are from hunting. If sex is also happening between Sam and Dean, or at least heavy petting, well... that's unrelated. And obviously Sam will deny it.
Anyway this ask absolutely isn't left over from last week and I will not stand for such slander.

Some people in the fandom go really hard on the "fuck me, [big/little] brother" thing and other authors have them not think about that at all (at least during the fucking). What's your philosophy?

See here's the thing. My heart says no, that it's just about each other. I don't think they'd be Like That if they didn't grow up in each other's pockets with no one else. So it isn't about being brothers, not really, it's about being each other's everything.
I can absolutely imagine the universe where Dean does say a lot of "brother" during sex... Right up until they discover Adam exists. And then he completely stops. "Brother" was hot and important to him because "brother" and "Sam" were synonyms. But now they aren't. I'd absolutely believe that happened.
That said, I might feel that in my heart, but a certain scene in the text makes the opposite argument. Going off that scene alone, there's a pretty damn strong argument that Sam has an incest kink, although I can't speak for Dean.
I doubt they don't think about being brothers during sex though. They are simply too into each other, and "brother" is a fundamental part of each one's definition of the other. I certainly don't think they're avoiding the incest thing. That said, because "brother" is such a fundamental and obvious part of each other, I doubt they actually have to say it or remind each other of it, either.
Let's be real. Having them say "brother" during sex isn't about characterization or what they would say in reality. It's about fulfilling the writer's own incest kink and how they personally think saying "brother" during sex is really hot. I think that is a good and noble purpose and people should keep doing it. But I'm also not going to think too hard about the realism there.
So, TL;DR: I don't think they have an incest kink as much as a Sam kink and a Dean kink, respectively... but Ruby might disagree.
