We all know that on some level the Joker actually does care for Harley but he also abuses her in many ways. 90% of the time she's just a tool for him. Ivy actually loves Harley and would do anything for her and also knows about her history with the Joker which she tries to help with. At the end of the day Harley and the Joker need each other in some way shape or form but Harley also needs Ivy whereas Ivy doesn't need Harley. Without both Harley would eventually lose herself too much and shut down. I mean she had the Jokers kid for christ sake. Harley is and always has been a symbol of entrapment in abuse. No matter how many escape paths she has she chooses to stay for a reason that no one else understands. Without the Joker she wouldn't exist and without Ivy she wouldn't still be alive.
