Yeah, they could have wired up some of the piano keys and used it like a ‘hitbox’.
Top: One of the most obscure hidden 1-Up locations in Super Mario Bros. 3 is to the right of the brick wall in the first outdoor segment of Level 5-Tower. However, note that at the end of the footage, after Mario flies off-screen again, a second 1-Up Mushroom falls past.
Bottom: This is due to there being two more hidden 1-Up Mushrooms in this room. Only one block appears on-screen as seen in the footage; but two additional ones are above the visible area, making them doubly hidden due to nothing in-game suggesting their existence. As getting these requires precise mid-air maneuvering without being able to see Mario, they might be the most difficult-to-obtain items in the game.

Forever Paccet is a tribute to Baraduke, a Namco-developed game released in 1985 for arcades. The first half stars Paccet, an alien, in differing situations; the second half introduces Baraduke’s protagonist, Kissy, who proceeds to go on an adventure with one Paccet and rescues more along the way.
In the Special World level Groovy in Super Mario World, there is a set of ledges with four Pokeys on them. By selectively having Yoshi eat parts of the Pokeys, it is possible to reduce all of them to only the head; then, once the Pokeys fall to the floor, they will overlap each other perfectly, appearing to be one Pokey.
Left: Demonstration of the four Pokeys appearing as one. Yoshi needs to lick it up four times for it to be defeated.

Interesting trivia: The third opening of Kimagure Orange Road shocked many animation otaku at the time of its release. In addition to being one long continuous take, it features several rotating shots, something very difficult and time-consuming to render via hand-drawn animation. This level of effort was practically unheard of for a weekly television series in 1988, and to this day it’s considered one of the more technically laborious anime openings ever made.
