Overkill or as any d&d player would put it negative hit points have been a thing since ad&d and only dtopped in 5e with death saves. Death occured at a specific negative number so big hits could instantly kill good god
Alright next BG1&2 were made by players really good at d&d, maybe true, i can't run a fact check I'm as valid a primsry source as CD is being. Which is to say not a primary source.

Ignore the original dragon magazines where Elminster while world hopping brought back Mt Dew, Volo making a spell to steal tarts, or the fucking CHOCOLATE GOLEM! All very serious
I could go on but if you want maybe look up (Spoilers BTW) Castle Amber where there is just a Boxing Ring in the middle of this castle where you have to fist fight a noble or a Massive Illusion Boulder meant to fuck with the party in White Plume Mountain or hell even in Tomb of (fucking) Annilation there are multiple small but at the end of the day cruel jokes the dm is meant to play on the players including a FAKE ENDING where the book TELL YOU to LIE to the PLAYERS and tell them they won. Dungeons weren't hard in a vaccum thats just mind numbing, adventures were a big part of that even short modules often gave token nods at a bigger story and many connected to bigger stories through a series of modules they were often mini open ended novellas with backstory and stakes and reasons beyond "its the video game what we are killing these goblins for"
All that aside, imma level with you dear reader, i would rather flagilate myself with my own tanned tongue instead of play an adventuer that is explicitly designed to kill the characters, coming from someone whose played a PC in ToA & run ToA as a DM they aren't that fun they are boring and annoying.
Challenge is fun! Challenge like getting surprised by a black dragon in the swamp outside a city full of undead is fun as hell even when you lose your character cause you're invested in your characters story and the story your dm is telling
"This dungeon will kill you cause I really don't want you to beat me" is an dming philosophy that sucks so much ass it could serves as colonic irrigation
The GM/DM has infinite power in a gameworld, they control time and reality, they are only limited by what the person wants to do, if they make every encounter 6 chromatic dragons of each element type 9 beholders and a youtuber with a crippling addiction to caffine they CAN but that will swiflty leave them without a game to play as everyone gets really bored and just stops showing up.
Maybe... think about... idk making a dungeon fun? Or encounters... ya know interesting and different? If you reading this are a writer... literally just write the set up to an issue and just leave it open ended. Maybe make some NPCs if your feeling fancy, spray in encounters in this forest/desert/temple/sewer that fit either the modus operandie of the problem causer bush bash bosh short game ready
