Don't get me wrong, I'm no body coach and I don't exactly know what constitutes as healthy or unhealthy, so maybe you can be ripped while staying healthy, bUT—

Story-wise,

I just love the idea of Muriel literally becoming softer as he not only becomes softer to the MC, but also to himself. He doesn't hate himself or hold his wrongdoings over his own head and torture himself with his thoughts, but he treats himself softly. He smiles more, he goes out to see friends because he knows he's worthy of their affection and attention. He goes out to do things he likes to do. He eats more.

And all that softness shows.
