Thinking about how low bg is kind of a medical emergency, in the sense that if not urgently treated it is life-threatening, but it doesn't feel like a medical emergency because the treatment is like, drinking juice and waiting 20 mins then it's fine. It's just another Tuesday.

just because your symptoms or the part of your body that you experience symptoms is thought of as funny or weird doesnt make your illness any less valid, real, or serious.

The answer is because type 1 is extremely poorly misunderstood in Japan. Due to the aging population crisis (28% of Japanese citizens are age 65 or older), type 2 is slowly becoming less stigmatized as a “fat people” disease, though it is still known as 生活習慣病 (seikatsu shuukanbyou), a “lifestyle disease.” However, since so many people are beginning to acquire type 2 through completely natural causes, people in the last decade or so are beginning to understand what it actually is, which may be part of the original author’s inspiration.
However, type 1 is still extremely uncommon–only 2 people in 100,000 are diagnosed with it every year in Japan. Adult incidence rates are increasing slightly, but it’s not enough for the disease to be well understood.
As a result, I suspect that the entire premise of 君の膵臓を食べたい (”I Want to/Let Me Eat Your Pancreas”), which, by the way, was originally a web novel and was made into a live action movie, is based on the author’s misunderstanding of what type 1 diabetes is. The illness, of course, is really just a plot device for a story about friendship, impermanence, letting go of hatred, and healing.
Tl;dr the girl is probably supposed to be diabetic, but it’s diabetes written by someone who has no idea what diabetes is.
