Everyday while doing eye exams I inevitably ask people to look āto the leftā. Sometimes they have difficulty understanding so I repeat āto the leftā.
I anxiously await the patient who bursts out in song like I do in my head each timeā¦. ;)

In taking the visual acuity of an adorable 5 year old boy today, instead of reading out each individual letter on the Snellen chart he read each line as if it was a word (or attempted to make it into one).

The Night Before Match Day - Revisited
Hard to believe it's already been a year since my turn at Match Day. Ā Being around the 4th years in the hospital the past few weeks has reminded me of all the excitement and stress this process brings. Ā It's been strange and exciting being on the other side of things this time.Ā Wishing all the best to the 4th years tomorrow and remember: it'll somehow all work itself out.

The Night Before Match Day
With one more sleep ātil Match Day, there are really no words in the English language (or French, for that matter), to describe how I feel.Ā Though these words donāt do the feeling justice, Iāll attempt to say itās a strange mixture of eager anticipation, fear, excitement, nervousness, but most of all, hope.
Tomorrow, Iāll know the results and, fingers crossed, theyāll be what I wished for.Ā But they very well may not be.
Which is why even though Iām feeling nervous today with all the uncertainty, Iām really not doing too bad.Ā Iām imagining both scenarios, matched and unmatched, and have various feelings regarding each.Ā But when I get stuck on a negative aspect of one scenario or another, I can still flip a mental switch and imagine the opposite outcome.Ā Tomorrow, that switch wonāt be able to be flipped anymore.
I know Iāll be okay with whatever happens, eventually.Ā Stuff will somehow figure itself out over time.Ā I may have to adapt my plans, try again, take a detour, but Iāll still get to my destination one way or another.Ā Iād prefer not to take a detour, but who knows, maybe the scenery along that road will be amazing and totally worth it.Ā As the Stones so eloquently put it, you canāt always get what you want, but if you try sometimes, you just might find, you get what you need.Ā (Go ahead, enjoy the tune.)
So fellow residency applicants, appreciate the last day of uncertainty for what itās worth.Ā All the best tomorrow.

Why N=1 Is Still Important
Fleming's discovery of penicillin couldn't get published today. That's a hugeĀ problem.
It is a well accepted scientific principle that the larger a studyās sample size, the more robust its conclusions. However, every new idea/question will always be sparked by an individual observation.
Thatās why case reports are vital to spur further scientific enquiry and share ideas.
Of course we canāt make evidence-based treatment decisions with case reports, but anyone who downplays their value does so at their own peril.

Raison D'Etre
Today was a really tough day at the hospital.
I was on call Friday and Sunday and got very little sleep last night as a result.Ā I was very tired both from lack of sleep and not having a day off in over a week.Ā Ā This current rotation has some different norms regarding paperwork than Iām used toĀ and I always seem to be filling it out wrong or miss writingĀ out information in a certain place.Ā And I donāt particularly like making mistakes or missing thingsā¦.
After completing what I hoped was all the paperwork needed for a patientās discharge, I went to speak with him to ensure he knew the plan and what was to happen next.Ā I wished him best of luck on his recovery and then he said some words that I will never forget:
āThank you very much doctor.Ā You have been very thorough, kind, and understanding.Ā I am very glad you chose to become a doctor because you are a very good one.Ā I wish all doctors were as nice as you.Ā I am so thankful for all you have done for me.ā
I was taken aback by his very kind words.Ā I really needed to hear them today.Ā It was a serendipitous reminder of why I got into this field in the first place.Ā Patients: the Raison DāEtre of every doctor and trainee. :)
Fitzpatrick Type 0
Ever since learning of the Fitzpatrick Skin Typing systemĀ in second year medical school, I've always been convinced Iām a Fitzpatrick Type 0.Ā
Yes.Ā Zero.
Doesnāt exist you say?Ā Well, during clinic today with a fully certified, attending/consultant dermatologist, he confirmed my suspicion.Ā At first he laughed it off and jokingly said āCome on, nobody is a type 0.āĀ But then he turned around from some paperwork, did a double take, looked at my arm, and saidĀ āWell...(pause, then in a serious tone), I think youāre right.āĀ

Long Time No Post
It has been a loooooooonnnnggg time since I've posted something on here.......
Since my last post I've:
Graduated medical school.
Moved to a new province.
Started residency.
Bought my first home.Ā
Exciting times indeed!Ā Iāve also changed the title of my blog andĀ TwitterĀ from Want2BeMD as, although I still āwantā to be an MD,Ā Iāve officially become one!
While it's been busy, I've also been rather lazy by postingĀ 140 character blurbs on TwitterĀ instead of full blog posts here.Ā Ā Each time I had a potential blogĀ post, for some reasonĀ it felt weird to re-start with that topic because Iād been absent from here for so long that, well, it just felt awkward.....becoming a self-perpetuating cycle.
Time to break it and start posting here-and-there again.Ā Stay tuned!
Do not regret growing older. It is a privilege denied to many.
