I have mixed thoughts on the line in particular and what it actually means, I've seen a few people say that Mickey actually meant he'd had 5 orgasms, not necessarily that they had fully penetrative sex 5 times. And I think that makes a little more sense to me and seems more in line with what would be more likely to happen in reality.
But they've always had such high sex drives and have always been so down bad for each other. Like at first when they hooked up you could say it was puberty and hormones. Like in season 1 when Mickey shows up at the Kash and Grab, but then a few hours later Ian is also sneaking into Mickey's room. Sex multiple times a day is definitely not a new thing for them. Even Mickey insisting they fit in another quickie before Ian goes to work the morning after the sleepover, despite his injury, shows us how insatiable they both are.
I also think it must have been such a freeing thing for them in the early days, to have someone to explore their desires with that they trusted to keep their secret. They had fun together and learned a lot about the kinds of things they would want for themselves and from each other. Sex has always been a form of communication for them and something they've never had to work at.
The fact that seemingly hasn't changed in a decade is a testament to just how compatible they are. But, I think it also did impact Mickey's response to hyper sexuality being a symptom, because Ian being insatiable to him isn't anything new or out of the ordinary. It's been that way from the beginning, long before Ian's bipolar manifested.
Like in season 5 when Ian says he can't sleep and wants to go again, and Mickey just kinda says "again?" in that sleepy voice and that teasing smile. Because to him this is just Ian, and par for the course. But the fact that Ian tells his family he 'wore him out' when he shows up to the pool party without Mickey is where I think the audience gets a glimpse that this might be something a little different. Because Mickey has presumably always been able to keep up before.
Throughout seasons 4 and 5 we can see Ian starting to spiral a little, and things mount up to a point where they collectively can't be ignored, even if they were excusable or able to be explained away individually. And I think that's why Ian doing the porno is when Mickey realizes that Ian really is unwell. Because going at it like rabbits is normal to them, working at the club because they need the money is also somewhat understandable as long as Ian's agency is intact and he's safe, but these things compounded with him then making reckless sexual choices with strangers without protection can no longer be explained away.
Sorry if I went off on a tangent a little, but I think it shows how far they've come to be able to be happily married and still so insatiable and mutually obsessed with each other sexually.
