Please help me get top surgery

I need to find a surgeon that will give me top surgery without me being approved for hormones. My therapist is a filthy piece of shit and she is ignoring me after describing me as “ball-less and brainless” and won’t give me a written diagnosis, which is necessary to get hrt and surgery. I know it’s not necessary in some countries long as you pay (which is not a problem). Can anyone help me find a place where I can get it? I can’t stand this situation anymore.
