If you are seriously asking and not just being a jerk it's pretty much the person using the accommodations. Doctors and the government also get involved.

There is literally no way to tell if someone is disabled simply by looking at their physical appearance. That person who just walked out of her car in a handicapped space? She may not be able to walk long distances without her knee pain flaring up and being unable to walk. The man who stood up off his medical scooter and walked out the door? He may not be able to stand or wall for certain amount of times without passing out. It's not something people can judge without the persons medical history.

I get that a lot of people are trying to be heroes and vigilantes who stop the fakers from mooching off all these accommodations, but more often than not you're harassing a disabled person who had gotten this same treatment many times before.

It's not your call, so just stay out of it.
