Reasons why I think o!ciel’s real name is Sirius:
Sirius is the name of the brightest star in the night sky, which also goes by the title “Dog Star”. It’s often tied to dogs in general. It’s a beautiful way to parallel the job of being the Queen’s Watchdog
r!ciel went by the name of “Lord Sirius” during the boy band arc. it would be just the right type of petty for r!ciel to parade around with o!ciel’s real name after his brother went years stealing his identity.
lmao adding to that is this fuckin line from chapter 169:
Sirius was thought of to be one star before astrologists developed strong enough telescopes and it was discovered in 1862 to actually be two stars. the parallel of this to the story is uncanny
a stretch, but Yana may have placed hints of his name as early as the Campania arc; the ship was sailing on the “Blue Star Line”; the larger star, Sirius A, is a blue-white star.

There are other mentions of this binary star system, including in the anime version of the Jack the Ripper arc, as was pointed out by @guarddogclaudia about… maybe two years ago:
However, it’s much better to use licensed translations of panels when discussing theories, since the licensed translations are more likely to keep the nuances of the original Japanese. Not to say the licensed translations are always perfect (I have found errors, and so have others, like Ducky), but they are more dependable by far than the various fan translations out there. Based on this (and everything else we know about real Ciel’s plan of “attack” against his twin), “Lord Sirius” is simply the title that real Ciel went by before he could reclaim his title as Lord Ciel, which Polaris calls him in the very next panel. Now “Lord Sirius” is meant to refer to his younger twin, simply because real Ciel and his peeps are now framing his younger twin as the leading perpetrator in the Sphere Music Hall killings. Polaris and everyone else now have to call our earl “Lord Sirius” in order to keep their cover-up story going. If Polaris were to accidentally call real Ciel “Lord Sirius” in front of Abberline or anyone else associated with the Yard or the London Police… or even the Midfords… then it would blow their cover.
As I mentioned in the linked post above, there are two particularly interesting things to note about the binary Sirius system:
The white dwarf Sirius B we see today used to be even bigger and brighter than the blue-white Sirius A. I used to say the white dwarf was our earl, but now I think it better represents real Ciel, since this bizarre doll version of him has “lost his radiance”.
The two stars are getting closer to each other, thanks to their balance being thrown off (or perhaps this is how they ended up being a binary system to begin with), and someday they will collide. This is good evidence (imho) that Undertaker will try to combine our earl’s soul with real Ciel’s body. It could also be seen as foreshadowing that their souls will eventually unite inside Sebastian, once the demon consumes our earl’s soul.
It might be best to say “Lord Sirius” covers both twins, since it’s been used to refer to each of them, plus it’s fundamentally two entities that are intrinsically connected… and I see no reason for Rachel to have chosen it as a given name for the younger twin.
Somewhere, in the Mother3 theory-addled recesses of my brain, I’m still holding a glimmer of hope for an anagram of Ciel… like Elic, which is a variant of names like Alex, Alec, and Alexis… which all basically mean “protection”.
