Lev is not transgender. Is only a thirteen years old girl who does not accept her role of woman in a rigid society like that Seraphites.

We can see that, when Abby asks Lev (Lily) why she shaved her head. She replies that she doesn't know.

Then after the plot goes on, we find out that Lily was supposed to marry one of the Elders. Here we talk about child brides.

Lily finds a way not to submit to this forcing and reaffirms it shaving her head (as Seraphites men do) thus managing to get out of her role imposed as a woman.

She is just a girl who rightly does not want to be raped for all her life by a man older than her and have children with him (also because I assume that she does not want to have children in the future, if she could choose).

A character that I consider transgender in a videogame is Krem, in Dragon Age: Inquisition.

If they really wanted me to believe that she was trans, they had to do it better as a character (since I consider transgender Krem). In addition, Krem is over 13 years old.
