-HUGE ASS MAP with a good blend of different terrains (endless wastes, demolished cities, hills/ridges/valleys, irradiated bodies of water) and plenty of locations to visit.
-Special Karma interactions, high/low karma could be used to intimidate people, charm/bully merchants into discounts, or lower speech requirements
-Improved character creation, tribal hairstyles, and more black hairstyles (3 and NV have like, what, one? lame).
-Also some shared hairstyles between gender categories would be great
-Body type customization. Scars.
-Ghoul perk/quest option, through which you can choose to ghoulify yourself and obtain certain benefits
-Keep the NV iron sights option, seriously, nobody wants FO3′s aim system
-More crafting and survivalist recipes/perks
-Keep NV Hardcore Mode option and increase decay on sleep
-VEHICLES/MOUNTS, I wanna ride a Yao Guai, or at least a motorcycle
-Improved melee and unarmed combat abilities
-Ability to create more custom weapons, more customs for existing weapons
-Keep the companion wheel and companion character quests
-Allow at least 2 or 3 companions to follow at once, plus a bonus pet
-More options on how to complete quests, the main quest especially should have a good variety of options
-Choices made during quests/interactions should have an effect!! (not like Skyrim where nothing you do matters)
-More factions, make it possible to redeem yourself once vilified (or allow negotiation with certain vilified NPCs). Also I wanna still see the Followers.
Bonus: It might be cool to have a first aid/stimpak animation, maybe.
