I understand, and because a court has stepped in there is a legal angle, but the reason trans people are/were prohibited from military service (for the last 245 +/- years) is a twofold answer.
I was an Army 79R Recruiter and Station Commander; most people who walk into a station leave unqualified for moral, medical, or mental reasons. Seventy percent of all applicants fail. 70%. I have had to turn away 3 trans people in my time as a 5 years as a 79R.
The foundational principle of military service is the shared burden of operational deployment. We all have to roll the dice for deployment and warfighting; it is a shared burden. Trans people will never share that burden based on medical reasons that prohibit deployment.
A complete medical and surgical transition is a multi-year process spanning 3 to 5 years, requiring multiple staged surgeries, extensive convalescent leave, and restrictive light-duty profiles. On a standard 4-to-5-year enlistment contract, a service member undergoing this process occupies a critical manning slot while remaining largely unavailable for overseas deployment. This shifts an unfair operational burden onto the rest of the unit, who must absorb the deployment cycles and field rotations for a position that is locked up on paper but ineffective in the field.
These transitions REQUIRE lifelong medical attention, every day for the rest of their lives. That medical care requires very specific medical devices and procedures that CANNOT be done over a long-term deployment on a shitty little FOB with just a Combat Medic for your day-to-day care.
The military disqualifies approximately 70% of applicants at the recruiting and MEPS level for minor, manageable medical conditions (such as a history of ADHD, asthma, or severe allergies requiring an EpiPen) solely to eliminate variables that could compromise a unit under pressure.

Regardless of the civilian debate surrounding the social or psychological reasons for this percentage, from a strict command and risk-management perspective, a 13% rate of fluctuation or reversal in a major, life-altering medical and psychological protocol represents a massive unknown variable. In a high-stress environment involving firearms, explosives, and complex machinery, importing a demographic with documented statistical rates of identity volatility and elevated psychological distress is a direct threat to predictability, unit cohesion, and operational safety.
The fiscal impact extends far beyond active duty. When a complex medical transition is initiated or managed during active service, the resulting lifelong care requirements become a permanent, service-connected liability for the Veterans Affairs (VA) healthcare system. The taxpayer is saddled with the lifelong financial responsibility for hormone therapies, surgical revisions, and mechanical implant maintenance for an individual who, due to the timeline of their transition, may never have been medically cleared to share the combat risks that VA care was designed to compensate.
This is not discrimination, this is the reality of military life.
