Ok well for that I think I'd probably start at like a lower mid point just to give myself room for momentum, then do sweeps straight down where one hand follows the other along the spine path so it's a one-two hit on the itches as I go.

Then once I do that a few times I'll mostly keep lower and scratch circles outwards to get any stray itches with either hand on each side that work back inwards to the spine and kind of climbing back up to the the start of the lower back, where I'll start scratching downwards again with both sets of nails facing to be scratching center spine.

Idea: Scratching the back of a person who's waaay too big to reach and scratch themself.
