Today I'm thinking about: a fantasy hero type who is captured by the bad guys just as he steps away from his party to go pee behind a tree, and tossed in a dungeon. He of course, being the hero, figures out how to escape right away. He does think about peeing first... but his party is probably worried about him so there's no time to waste.
Though he gets out of his cell easily enough, actually escaping the bad guy hideout takes a lot longer than he expected. He's got to sneak past patrols, fight guards and monsters, and as it goes on his urge gets worse and worse, and he starts actively looking for somewhere he can pee without a guard sneaking up on him.
But there's nowhere, not even as his standards get lower, and by the last couple of fights he's leaking into his trousers because he can't focus on holding it and combat at the same time.
Finally he gets outside, and is just about to let go and piss on the wall right there, but he hears a guard patrol coming and realizes even here isn't safe. So despite another big leak he leaves the hideout and makes his way back toward where he lays saw his party, looking longingly at every tree and corner and big rock he passes.
And when at last he gets far enough that he thinks he's safe - or honestly, just gets to the point that he knows he has to piss now or he's going to wet himself - his party suddenly bursts out of the woods and runs up to him, asking what happened and if he's okay. The hero tries to reassure them that he's fine, really, he's not hurt, but if they could all step away for a second, or at least turn around? But it's too late, his bladder gives out, and he completely pisses himself right there in front of all his friends and allies.
