Graduates high school and moves to Hyogo Prefecture, where she immediately enrolled into an undergraduate medical school.

Yoshio pays for her school and living expenses, but Setsuka took on small part-time work in a hospital to get a feel on how it functions in general.

Stayed in the prefecture for around seven years before moving back to Namimori and becoming an emergency doctor at the local hospital before eventually finding another job a couple years later as a primary care physician.

When she became a primary care physician in a private practice, Tsuna and his guardians became regular patients of hers at the practice ... and sometimes in her apartment.

Eventually opens her own private practice where she also makes homegrown medicine.

Tsuna and the guardians naturally move with her to her private practice and are regulars when a mission is particularly difficult to handle and injuries are quite spread out.

She unofficially became the family doctor, eventually having her friends come to her for check-ups and problems. Not that she minds because she's happy to assist them ... even though she wished they go to a proper medical facility for more serious matters after the more violent missions.

For funsies, i also made a list of how she reacts to the guardians.

hibari being surprisingly docile when he does his checkups with her. Hes also pretty good at taking care of himself, so she never really worries about him.

Chrome usually has her worried because of the condition of her body and her eating habits, but she's been getting better with Kyoko and Haru's help

Gokudera, he probably worries her because lbr, he eats a lot of convenience store food I bet judging from the anime.

Ryohei and Takeshi are both athletes, so theyre both relatively healthy people and eat a good balance (takeshi i bet cooks for himself, ryohei eventually learned how to cook for himself to keep up his physique like takeshi), so she doesn't worry THAT much.

I dunno if Mukuro would go to her, he seems like the type that doesn't wanna deal with the vongola if he doesn't have to... or if he does, he's surprisingly docile as well.

Lambo, Ipin, Bianchi and Futa all probably still live with Nana and they're usually of good health because Nana takes care of them

Tsuna, I imagine sometimes skips meals at times if hes busy, so she’s usually on his ass about eating properly. He eats good when he stays over at his mom's though, but he's probably clumsy in the kitchen on his own

Kyoko and Haru are pretty good at keeping track of their health most of the time, so she never worries about them either.

We don't talk enough about how Desscaras forced everyone's hand by signing the mentor-pupil blood contract. Like the other witches seemed distressed by the news that Ichi would likely have to die, but from their perspective Ichi is like...a fun anomaly. He has no family and no connections. He's not properly trained. He has no sense of the future and only cares about hunting. Ichi dying to take out the world-hater majik is a total non-issue in the grand scheme of things. They lose nothing. Easiest trolley problem ever.

Honestly with what we've learned about Jikishirone in the most recent arc, I'd say she gave this little speech specifically because she knew it was going through everyone's heads and she wanted them to confront the thought fully. Monegold gets angry about it, and I don't think Monegold is necessarily a bad person, but she does have to think about things in a broader "what's good for everyone" stance as the leader of the association.

And then this absolute madwoman Desscaras turns around like "oh, you thought you wouldn't lose anything by relying on your easy sacrifice? Well now if he dies, so does your #1 best witch ever." She literally took the option away from them. She's decided that she didn't want to rely on a kid's death to save the world, and now she's made it clear to the higher-ups that they ALSO can't do that unless they want to lose her. Save her kid or watch your association crumble. She's so right she really is the greatest of all time.
