Diavolo and Doppio don't actually look that different but they both experience a light body dysmorphia/distorted body image given the fact that their personalities are very split apart (sometimes we don't directly talk about it but they're headmates, and how aware they each are of this is kind of ambiguous), plus they both have different beauty rituals and fashion style so to anyone that doesn't know them, it's easy to tell them apart as different people because there's no point of comparison
