An old cowboy, in boots, hat & severe pain. Loose bridge to complicate everything,28/29 nec. aa. Accessed, cleaned and medicated, had suspicion of canal bifurcation from pre-op. Took scan after medicating, both with splits. Had difficulty with loose bridge and sealing internally and locating canals, especially in 28. Great guy in a lot of pain, hadn’t slept in a while. He was snoring in chair while I was working. 3969. Nec/sap. Had some int. resorption in distal of chamber. She wasn’t happy. Pre-op PA of unflattering endo tx. Decisions, retx w Ca Hyd to arrest inflam resorption, etc. Or tx surgically? CBCT , sag view. I could medicate forever, it wouldn’t heal. Surg all the way, or extract. She said extraction wasn’t happening. (4108). Post - op PA then 15 month recall PA. She’s happy. Nice lady presenting w indurated buccal swelling and mild aching (its “better” after a round of antibiotics). Finding on mb apex of 30. Obturation shows transportation, but not a significant as 31. Perio intact. Bottom line, re-tx, surgery or ext/implant? Can the fill be improved? Not likely, so surgery or implant. How to help the decision? Gotta have a scan. Pic 2 displays a series of 1.13mm slices and a progression of half moon shaped lucency from furca (along mb surface) to apex, where the cancellus bone is destroyed. I treated this case in 5/2014, sig bone loss, cap, etc. calcium hydroxide until sinus tract healed. Temp w cotton pellet and Cavit. 😑(2022). She returned 4/17 and had not had permanent restoration placed. 🤢. I cleaned (& cleaned) & cleaned and placed a core and sealed.
