You know this is a really interesting depiction of the 4 of swords. Usually I see the 4 of swords as a card of rest after a heartbreak is over from the 3 of swords ending and moving into the next phase.

Due to the fact that the wolf still has the 3 swords sticking out of his back, it depicts to me that he has had no choice, but to give up and rest after defeat from the wound to the heart. Furthermore, the sword lying in front of him shows that he has put down his sword and isn't fighting anymore.

This 4 of swords is truly a card of surrender to circumstances, giving up for a while before moving into the next phase. While the typical meaning of this card is a willful rest and recuperation, this one shows that there was no choice, but to rest because of the pain.

But this period of surrender will allow him to move more easily into the next phase. The 5 of swords where the understanding of deceit and hurt begins to be understood and explained.

Lastly, the moon in the background represents the moon card to me, things that are unknown that still haven't come to light yet, from his past because the moon is in behind, him. Things may be partially illuminated, but he is still not able to see the full picture, which could be due to his pain, or just because he's not meant to understand quite yet in this phase.
