Listen we all know he murders people but we are judging him purely on his medical expertise

Guy with intentionally negative bedside manner because he's so firm and confident in his ability to fix the problem that he refuses to deal in platitudes and has one of the highest patient success rates of all time vs malignant cannibal who even without the cannibalism does the opposite of therapy.

Again though the eating people isn't really relevant. This isn't "who is a worse person" is who's a worse doctor. House cures like 99% of his patients, even if through weird malpractice. Hannibal does not. Even if he never ate anyone he's the worse doctor. If I was choosing a doctor, I'd choose House, and just booby trap my front door or something.
