Understanding Sizes
Part 1 : The Nipple Size
Pacifiers come in different sizes. Newborn pacifiers use NUK 1 nipples, 3-6 month use NUK 2, 6+ use NUK 3 and 12+ use NUK 4. Adult pacifiers usually use NUK 5-7.
NUK 5-7 have longer necks of the teat and wider heads, making them easier on the adult jaw and allow the pacifier to sit properly. What you prefer is about mouth size and preference .
I actually prefer NUK 4, as I have a shallow mouth, so I need to modify an 18+ month teat onto adult shields.
Part 2: The Shield
Infant and toddler shields don’t cover the adult mouth, which is really an aesthetic issue only.
Adult pacifier shields cover the whole mouth and sit more fully.
Dealing With Myths
No matter the size, pacifiers will not harm your teeth. What can harm them is excessive usage and sucking on the pacifier, but this would require a lot of time and pain would be the first indicator long before damage. This is a concern mainly for children with milk teeth, where long use pacifiers can cause disforming of the incoming adult teeth.
The real concern of using the wrong nipple size is jaw tightening. Because infant pacifiers do not sit properly, an adult has to bite more to keep it in, keeping the jaw at an awkward angle, which can lead to jaw pain and headaches.

@littlealpacacutie The other thing to consider when it comes to baby pacifiers not covering the full adult mouth is risk of choking, especially when used during sleep. Adult pacis are much safer to use at night because the bigger sheild keeps them from going where they aren’t supposed to.
I also want to say that if you have a high pain tollerance, frequent headaches, or have had braces in the past, it may be hard to pick up on the signs of your teeth moving!
One of the reasons I use adult pacis was because I thought it might be able to help with my underbite. (I do not reccomend trying to treat an underbite this way, but I’m broke, and wanted to use pacis anyway, so I gave it a shot.) My teeth have moved, but at first, I didn’t notice it.
This is because, having already had painful braces and a high pain tollerance in general, I didn’t notice the pain of my teeth moving.
For me, the pain manifested in headaches before the teeth themselves hurt, so if you have frequent headaches, you may have a hard time telling the difference between your normal headaches and the tooth-moving headaches.
The best thing to do is to be careful! Watch for pain, but also watch for changes in the alignment of your teeth! Get to know what your mouth looks and feels like pre-paci and check to make sure it stays that way!
Moving teeth permanently takes time. If you move them a little bit, they will go back to normal assuming you give them a chance to.
If you notice your teeth have moved, discontinue use of the pacifier until they have gone back to normal. If your head, mouth, jaw, and/or teeth hurt, discontinue use until the pain has stopped.
