This could be a parallel to the helicarrier scene in catws. After he's made sure the world will be safe, he tells Maria to destroy the helicarriers, even with him still on one. Because he's so tired and the one person he loves more than anything doesn't know him, so what's the point? At least this time he can die with Bucky, like it should've always been. But then he hears Bucky scream, and looks down to see him pinned by a falling beam. So he goes to help him.
Steve, with several bullet knife wounds, jumps down and lifts the beam off the person who just gave him those injuries. The amount of strength it would've taken to do that is insane. And Bucky might just try to kill him again. But you know that, hearing him scream and seeing him look so scared, he wasn't thinking of the Winter Soldier. He was thinking of his friend, the person he had even when he had nothing. His friend needed help, so he decided to try one more time. Because he can't stand living in a world where he couldn't save Bucky. Even if Bucky killed him, it would be okay.
Twice Steve helped Bucky even when he was so exhausted and injured and I'm so normal about it.
