Esme's blood is the most incredible thing Astarion has ever tasted. But he only fed on her once. She uh, died.

So the morning after (post walk of shame to Withers), Esme tried to avoid the subject. But also said she doesn't want to do that again and that he can feed on enemies. Esme being very sweet about it, but very visibly hurt by the ordeal affected him more than he realised. She trusted him, and he killed her for it. Even then she never once held it against him.
