4th day of HRT

No dose changes. I’m considering to increase my Spironolactone dosage in the foreseeable future, though.

It’s only 4 days and I can feel that my skin feels a little bit softer, especially in the areas sensitive to E. My hands get tired easily and my sense of touch has been changed a little. I can control my upsetness and anger more easily, but I feel them more intensively at the beginning. I feel my skin is dried a little bit, but my face is still very oily indeed.

Generic Available

Common Dosage Forms:

Tablets: 25 mg, 50 mg, 100 mg

FDA Indications/Dosages:

Edema due to congestive heart failure or nephrotic syndrome and cirrhosis of the liver accompanied by edema and/or ascites: Initially 100 mg/day with adjustments at 5-day intervals to a maintenance dose of 25-200 mg/day.

Treatment of essential hypertension: 50-100 mg/day.

Treatment of hypokalemia: 25-100 mg/day.

Diagnosis of primary hyperaldosteronism: Long test: 400 mg/day for 3-4 days. Correction of hypokalemia and hypertension concludes a positive test. Short test: 400 mg/day for 4 days. An increase in serum potassium with a subsequent decrease after the fourth day concludes a positive test.

Treatment of primary hyperaldosteronism in patients unsuitable for surgery: 100-400/day.

Pharmacology/Pharmacokinetics: Spironolactone works by competitive antagonism of aldosterone, primarily at the aldosterone-dependent Na+/K+ exchange sites in the distal convoluted renal tubule. It reaches peak plasma levels in 3-5 hours, with a half-life of 2-16 hours. Excretion of metabolized drug occurs in the urine.

Drug Interactions: Hyperkalemia may occur when used concomitantly with captopril and with potassium-sparing medications, including potassium supplements. Variable effects may occur when used with DIGITALIS glycosides.

Contraindications/Precautions: Contraindicated in patients with anuria, acute renal insufficiency, significant impairment of renal function, hyperkalemia, and in patients hypersensitive to sulfonamides or spironolactone. Use with caution in patients with acute or severe hepatic failure. Patients receiving this medication should be monitored periodically for fluid and electrolyte disturbances. Use with caution in pregnant and in nursing women. Pregnancy Category B.

Adverse Effects: Diarrhea, drowsiness, headache, amenorrhea, WEAKNESS, PARESTHESIA, MENTAL CONFUSION, DRY MOUTH, MUSCLE CRAMPS, hirsutism, urticaria, impotence, gynecomastia, nausea, and vomiting.

Patient Consultation:

Timing of dosage is important because of increased diuresis. Usually given in the early morning.

May be taken with food of milk if GI upset occurs.

May cause drowsiness. Use caution while operating machinery or when mental alertness is required.

Store in a cool, dry place away from sunlight and children.

Contact a physician if the above side effects are severe or persistent.

If a dose is missed, skip it and return to normal dosing schedule.

I’ve elected to forgo the full body image as there aren’t any noticeable changes at this point.

Physical - 265 lbs, 5′9″, 23 - 1 month on 25mg Aldactone

- Skin I have noticed a slight decrease in acne and an improvement in overall skin health, but Aldactone is an acne medication, so this is unsurprising.

- Hair (body/head)    * Hairline: No change    * Texture: No change    * Thickness: No change - Face    * Hairline: No change    * Cheekbones: I may be looking for changes, but I feel like my cheeks are just a little fuller than they were a month ago...    * Facial hair: No change    * Eyebrows: No change    * Eyes / Eyelids / Eyelashes: No change    * Acne: Minor decrease    * Skin type (oily, dry, etc.): dry (no change) - Body : NO CHANGE    * Fat redistribution (shoulders, hips, calves, breasts, etc.)    * Hair growth (thinning, lightening, etc.)    * Libido (lower libido, do different things turn me on, etc.)    * Testicular shrinkage    * Temperature resilience    * Nails?    * Changes in cravings / taste/ favorite foods?

Mental/Emotional Mental health check-in: - Anxiety: I have had an increase in anxious days / anxiety attacks, which I can only attribute to physically-manifesting dysphoria - Depression: Increase, but is following my typical depressive cycle  - ADHD (?) - Dysphoria: Clear increase, but like I said above, taking Aldactone inherently requires me to actively focus on my gender more often than I used to, which leads to noticing misalignments.  The worst days so far are (like usual) the days when my beard has grown too long and is starting to be noticeable but I don’t shave because of a lack of motivation or energy and I start to lose Alex. I’ve seen an increase in headaches, but I imagine this is due to dehydration as learning to hydrate regularly is an uphill battle.  In undergrad, there was a point where I was so super focused on classes (etc.) that I actually didn’t drink anything for three full days and almost passed out on the way to class, so I’m working on that.  I’ve also seen a general loss of energy; I crash much earlier in the day than I used to, but it’s a noticeably different tired than depression.  Though my depression has more or less stayed the same, bad days are even more likely to see me in bed for 24 hours at a time. Finally, I’ve noticed an increase in dysphoric days, but that is to be expected as actively committing to HRT inherently brings gender to the forefront so I’m more likely to notice incongruences than in the recent past, when I had elected to bury all gender-related emotions.

Concerns I dyed my hair a fairly normal color today, which I haven’t done in at least a year and a half, but I’m going to have to start applying to jobs soon and I’m starting to get concerned about my prospects.  Of course, as someone who will have a master’s degree by then, I will be highly qualified for most jobs I come across and feel confident that employers will see that, but I don’t want my gender identity to get in my way?  Which is part of the reason I am only taking Aldactone for now, because I want to hold on to that “male privilege” for a little bit longer, but it almost feels like I’m simultaneously pushing myself further in and out of the closet.  The plan right now, though, is to wear my hair up (maybe a french braid with the tail tucked) so it looks pretty short and by then my roots will have grown out somewhat so it’ll look mostly my natural brown, too.  I won’t wear too much makeup, but I’ve decided that I’m going to give my clothes a more feminine twist because I will feel way more comfortable which will increase my chances of an acceptance, and will leave social wiggle room to transition a little more smoothly (I hope) in the future. My worries have a stronger bite now, possibly because I am Doing Something, which is definitively different from Not Doing Anything and requires the active decision to continue pursuing this path every day.  Being genderfluid, I still have boy days, and demi-boy days, and boy-leaning androgynous days, so I have to remain willful and remind myself why I’m doing this.  How easy would it be to stop?  Just forget to take my medication one day and things would return to baseline, maybe I could push things away again, but for how long?  How long until it becomes a dire need to transition?  I have those days, where I Need to transition but the process doesn’t speed up because you want it to, so I’m holding tight and staying course.  I will see this through the next year and hopefully look back and praise myself for starting so early, even if only a year or two, because these next couple years are when signs of aging really start in men and I’m not here for it.

Update: 1/29/19

I'm on such little Aldactone that I don't want to misread placebo effects as true effects (meaning I'm going to have to start taking measurements [which, duh, why didn't I do that at the start?!])

but I've definitely seen a decrease in acne and my blood pressure is doing wonky stuff (imo a sign that it's working; given its half life of two hours, it's mostly out of my system within 6 hours, and that's about when I can feel my blood pressure spike), so knowing that it can cause gynecomastia might be enough to make me see growth, and I genuinely can't tell if it's new or just fat that I'm learning to carry more femininely, but color me hopefully optimistic because I feel like I see very minor breast and (left) nipple growth 😁😁
