🧠 Key Insight This case report presents a 13-year-old boy with chronic massive osteochondritis dissecans (OCD) of the knee, initially misdiagnosed and later found to have a large osteochondral defect associated with a discoid meniscus. Surgical management included osteochondral fragment fixation using bioabsorbable poly-L-lactic acid (PLLA) pins along with meniscal correction. Despite the chronic nature of the lesion and fragments consisting largely of cartilage, successful bony fusion was achieved within months, with significant functional improvement and return to sports by 6 months. The case highlights that even chronic and extensive OCD lesions can heal effectively with stable fixation, and bioabsorbable implants offer the advantage of avoiding implant removal while supporting cartilage repair.
