Wilson doesn't like to mix pain and work. So, if he's with a patient while he's feeling more pain than usual, he'll wait until the visit is over to take anything. He's usually good at masking the pain/not letting it bother him (chronic means that he's used to it by now) but complaining to House when it's just the two of them around also helps ease the pain, even though House doesn't see it that way.
If it hurts that bad, then take something. I can always go down to the pharmacy and get you something stronger; Vicodin even...
