People like to say that people knowing both identities of a superhero but not realizing who they are is stupid, especially when it comes to heroes like Supers who don't wear masks. And when Kara's literal best friend, who often works with Supergirl, doesn't realize it, people think it's unrealistic. But I don't think it is. I think that it clearly (even though it's never explicitly stated) shows that Lena is so, so, so mentally ill. To the point of extreme delusion. With her incredible brain constantly making connections, there was absolutely a part of her that knew. When she and Brainy talk in 4x08, Lena explains that she keeps her feelings in "imaginary boxes" to be able to function. A few episodes later, Brainy uses this technique to "forget" about Kara's identity. This is presented as almost humorously extreme, but it's what Lena does every single fucking day. She has been so traumatized for most of her life, especially by her close personal relationships, that her brain decided that the best way to protect her and keep her functioning outwardly was to separate a whole part of her reality from her consciousness. When Lex told her, that just stretched the barrier so far it finally broke. So, even though those different parts of her mind are connected now, I think that she seriously needs some intensive psychotherapy. What else might be in there?
(This isn't completely relevant, but since we're talking about the psychological state of Lena Luthor: she def has borderline personality disorder.)
